Sri Lanka High Commission in Application for a new passport Required documents Application form “K35A” (download here)
Old passport or copy of the old passport
Valid original Indian Resident Permit/ Legitimating card if available
Four (04) color photos 4.5 x 3.5 cm (without white boarder)
Original birth certificate or copy
Original National Identity Card or copy
Original Marriage Certificate if the applicant is married.
Photocopies of all the originals
Self addressed cover with stamp if the new passport is to be sent to the applicant by post
It takes about 08 weeks to issue the new passport. If applying for a new passport/travel document in case of emergency, in lieu of a lost passport, the original Police report with its English translation and duly completed Form ‘D’ should be submitted. Non Machine Readable Passport (NMRP): NMRP will be issued to Sri Lankans in lieu of a lost passport, for the purpose of return to Sri Lanka. The application should be submitted with the following documents: Indian Police report for the lost passport
Application form “K” (download) Form “D” -3 Copy of the old/lost passport (if available) Valid original Indian Resident Permit/ Legitimating card (if available) Four (04) color photos of 4.5 x 3.5 cm (without white border) Original birth certificate or copy,( if available) Original National Identity Card or copy,(if available) Any other document to prove the Sri Lankan nationality Self addressed cover with stamp if the new passport is to be sent to the applicant by post Change of Name in the passport after marriage

Consent letter of Husband
